Changes to Long Term Condition and Annual Review Appts

We are making changes to the way we provide appointments for you to be seen in surgery for your long-term condition and annual nurse reviews.

We have been successfully offering self-book appointments to our patients for their routine and seasonal vaccination appointments for some time now. We are now able to offer this facility to you to book your initial long-term condition or annual review with our nursing team.

When your review is due, you will receive the invitation message as usual, but with a self-book link. This enables you to select an appointment at a day/time suitable for you without the need to call the surgery. If you require a follow-on appointment, this will be booked for you by the nursing team when you attend for your initial appointment.

May we remind you that if you use the self-book facility and have in the past been requested to attend with a urine sample, this request remains, and you will need to collect a urine sample pot from reception before this appointment. Thank you

Of course, you are still able to phone into our Reception team who will be happy to assist and book the appointments for you. We are simply providing additional patient choice here.

Managing appointments during the busy winter months

The winter months are always extremely busy here in Surgery, and at times our nursing team are stretched for appointments. We have therefore taken the decision to invite those patients with birthdays in the winter months of December, January, February and March earlier than normal to help us to ensure you can be seen in a timely fashion. Please book your appointment as soon as you receive your invite to ensure the greatest choice and availability of appointments. It is OK to book an appointment one month earlier than your birthday month – this will ensure your medications are reviewed in plenty of time such that your medication ordering is not disrupted.
