Changes to Long Term Condition and Annual Review Appts

We are making changes to the way we provide appointments for you to be seen in surgery for your long-term condition and annual nurse reviews.

We have been successfully offering self-book appointments to our patients for their routine and seasonal vaccination appointments for some time now. We are now able to offer this facility to some patients to book your initial long-term condition or annual review with our nursing team.

If you meet the criteria we can use for this service, at the time your review is due, you will receive the invitation message as usual, but with a self-book link. This enables you to select an appointment at a day/time suitable for you without the need to call the surgery. If you require a follow-on appointment, this will be booked for you by the nursing team when you attend for your initial appointment.

Some patients are required to bring a urine sample with them to their review appointment. If you are aware this applies to you, please pop into the surgery to collect a urine sample pot prior to your appointment. If you are unsure whether this applies to you, please either pop into Reception who will advise or alternatively call our reception team who can check for you before you make a special journey into the surgery to collect the pot. Thank you for your help with this.

Of course, you are still able to phone into our Reception team who will be happy to assist and book the appointments for you. We are simply providing additional patient choice here.

As ever, your feedback is appreciated, so if you have any comments on this new process, please let us know by filling in a Friends and Family form in surgery or completing the online form on our website.

Thank you for helping us manage this change.
