Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
- use the NHS: register with Chipping Surgery service
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please allow up to 7 working days for your request to be processed and completed by the surgery.
Patients from overseas
Registering a child under the age of 18 – please ensure you provide us with a copy of their immunisation record as soon as possible to enable this to be translated (if necessary) and reviewed by our nursing team to assess any additional immunisations that may be required.
Please either bring this into the surgery or email us on chipping.admin@nhs.net.
